As Nottingham University Hospitals NHS Trust (which encompasses the Queen’s Medical Centre and Nottingham City Hospital) continues to face scrutiny and criticism for its poor maternity care via the biggest maternity inquiry in NHS history, we review yet another tragic case of preventable baby death.

The family of 17-year-old Finlay James Turner-Smith have spoken of their heartbreak following the conclusion of an inquest into his death. The inquest into his tragic death on 23rd March 2024 – held at Nottingham Coroner’s Court on 26 June 2025 – raised serious concerns regarding the emergency medical response provided by East Midlands Ambulance Service (EMAS).

An inquest was recently held into the death of Jonathan Baker, which found a number of missed opportunities and failings by healthcare professionals at Nottinghamshire Healthcare NHS Foundation Trust led to Jonathan taking his own life.

Losing a loved one is one of the most emotionally painful experiences. For many, however, it is even more so when that loss is sudden and/or unexpected, or where it is unclear as to exactly why they passed away.

An inquest was recently held into the death of Tracey South, which found multiple missed opportunities by medical staff at King’s Mill Hospital led to failure to rescue the patient.

An inquest will be held this week (beginning Monday, August 22nd) into the death of a baby boy shortly after his birth at the Countess of Chester hospital.
